It’s Benefits Open Enrollment time!
The Open Enrollment will be held from Monday, May 4, 2026 through Sunday, May 17, 2026

As we prepare for this year’s Open Enrollment, we are excited to introduce a new way to help you make informed decisions about your benefits – Benefits Fairs!
These fairs are designed to give you direct access to your benefits providers, along with helpful resources & guidance to understand your options when it’s time to enroll.

 

Open Enrollment is MANDATORY.  Action will be required by all Full-Time Team Members.  

All benefit eligible team members must complete the enrollment process, even if they wish to waive benefits.

Team Members who do not submit their enrollment by the deadline will have their current benefits terminated effective June 30, 2026, and will not have any active coverage in the new plan year.

No exceptions will be made unless the team member experiences a qualifying life event (see the Benefit Enrollment Process page for details).

Open Enrollment Highlights & Updates:

  • Dental – No rate increase for 7/1/26 – 6/30/2027
  • Vision – New enhanced benefits starting 7/1/2026 – higher allowance for Frames & able to use allowance for Frames every 12 months
  • Paytient – allows employees to pay any medical, dental, vision & veterinary expense over time, interest free
    • Note: This will only be available to those that are enrolled in medical coverage as of 7/1/2026

Company Paid Basic Life & AD&D Insurance

All full-time team members will receive 1x their annual salary rounded up to the thousand in company paid life insurance at no cost! The benefit carries a minimum of $30,000 and maximum of $200,000 in coverage. Benefit includes reduction of coverage after the age of 65, see Highlight sheet below.

Voluntary Life Insurance & Accidental Death & Dismemberment (AD&D)

Team members may elect voluntary life insurance for themselves, their spouse, and eligible dependents (up to age 26).  Team Members must elect coverage for themselves if they wish to elect for their spouse or child.

For New Hires in their benefit eligibility window &/or during a Qualifying Life Event (QLE), Team Members can elect up to $150,000 for themselves and up to $30,000 for a Spouse without having to provide evidence of insurability (EOI).  These are “guaranteed issue” (GI) amounts.

For requests above the guaranteed amounts, a completed EOI form is needed and coverage is subject to underwriting by Mutual of Omaha.

During the annual Open Enrollment period, Team Members that currently have this coverage, may elect up to an additional $20K for themselves and up to $10K for spouse (if applicable) without having to do the EOI form as long as the total amount does not exceed the GI amounts.

For those that are electing coverage for the 1st time during the OE period, any amount of coverage will require an EOI form to be completed & submitted to Mutual of Omaha for approval.

Coverage Specifics:

Employees – Must elect in $10,000 increments up to a max of $500,000 (or no more than 10x your annual salary)
           Note: Insurance benefits and guarantee issue amounts are subject to age reduction starting at age 65

Spouse – Coverage amounts must be elected in $5,000 increment up to max of $150,000 (or up to 100% of Team Member’s benefit amount)
          Note: Coverage for a spouse must end when they turn 70 years old

Dependent – Minimum/Maximum coverage amount is $10,000
          Note: Coverage for Dependents does not require EOI.  You pay one rate for all eligible dependents.

Short and Long Term Disability

Short-term & Long-Term Disability

Short Term Disability

Short Term Disability (STD) insurance is designed to protect an employee’s income for a short period of time in the event an employee becomes ill or injured and is unable to work up to 13 weeks. Benefit amount – 60% of your weekly salary for up to 12 weeks (following a 7-day waiting period). See full plan description for specifications and limitations.

Long Term Disability

Long Term Disability (LTD) insurance is designed to protect an employee’s income for up to five years after the short-term disability period has expired (at least 90 days from event).

    • Benefit amount – 60% of your salary for up to maximum benefit period (see plan highlight below)
  • LTD Highlight sheet – Class 2 (All active, Full-Time employees)
  • LTD Highlight sheet – Class 1 (All active, Full-Time Management employees)

Notes:
Team Member premiums are calculated based on amount of coverage elected and then based on your age band.
Spouse premiums are also calculated on amount of coverage elected, and then based on the Team Member’s DOB/Age Band.
If electing coverage for dependent child(ren), a separate premium is NOT required – there is no cost to Team Member for this coverage.  Premiums will be $0

Hospital Indemnity

 

FYI – Each of these supplemental insurance plans has a Health Screening Benefit attached to them.  Team Members are eligible to complete the claim form below to request reimbursement of $50 for having a health screening done.  Once the form is completed, it can be submitted to Mutual of Omaha for processing.  Team Members can file a claim 1 time per insured, per calendar year; up to max of 6 covered family members per year.  See the plan highlight sheets above.  Any questions, please contact Mutual of Omaha directly at 1-800-228-7104 (www.mutualofomaha.com).

Health Savings Account (HSA)

Health Savings Account Vendor: Inspira Financial(formerly Payflex)

What is an HSA?  Team Members who enroll in a High-Deductible Health Plan (HDHP) may qualify to open an HSA (Health Savings Account).  You can use pre-tax funds, to be deposited into an account to pay for out-of-pocket, eligible healthcare expenses. Unspent money is portable and rolls over into the next benefit year to cover expenses or to continue earning interest tax free.  HSA’s differ from an FSA in that you can take your money with you if you switch employers, or even if you change health insurers.  As you add funds into your account, your contributions reduce your annual taxable income.  The funds you withdraw to pay for qualified medical expenses remain untaxed and you save again when funds in your HSA accounts earn interest tax free.

Team Members can start/stop or change their contributions at any time during the year.

HSA limits are set by the I.R.S. each year and contribution limits are based on calendar year (January – December).
The contribution limits for 2026 are as follows:

  • Individual: $4,400
  • Family: $8,750

For those that are of Age 55 and older, there is an additional $1,000 catch up to the amounts listed above.

Flexible Spending Accounts (FSA)

Team Members have option to participate in two Flexible Spending Account options:

Health Care-FSA:

Team members may participate in the medical reimbursement account and set aside pre-tax dollars that can be used for medical, dental, vision and other eligible expenses as outlined by the IRS for you and your eligible dependents. Funds are eligible for use as of the plan year (July 1 – June 30th) or the Team Member’s benefit eligibility date – June 30th.  Contributions will be deducted pre-tax throughout the plan year from your paychecks.  Deductions amounts will be based on the number of pay periods left in the plan year based on benefit eligible period.

FY 2026-2027 (July 1, 2026 – June 30, 2027) 

Limit: $3,400
Minimum election amount required: $50

  • You may rollover up to $680 (from FY 25-26) of unused FSA dollars into the FY 26-27 plan year
  • You do not need to elect in the new plan year to be eligible for your rollover funds, but rollover funds will be forfeited as of 7/1/2027 if not spent for eligible expenses by 6/30/2026.

Dependent (Day) Care FSA:

Team members may participate in the dependent care account and set aside pre-tax dollars to help pay for childcare expenses. Funds are eligible for use once every payday. This plan is a use it or lose it benefit. Any unused funds in this account will be forfeited at the end of the plan year.

FY 2026-2027 (July 1, 2026 – June 30, 2027) 

Limit: $7,500 (per household)
(or $3750 if you and your spouse file separate tax returns).

We understand that pets are part of the family and that your pet isn’t “just a pet.”  Sun Health partners with United Pet Care to offer a pet benefit to support you in caring for them. United Pet Care is the affordable pet health savings plan that works for all pets!

United Pet Care is the affordable pet health savings plan that works for all pets.  It’s not a pet “insurance” plan where you can encounter issues with pre-existing conditions, etc.  Instead by visiting an “in-network” primary care vet, you can save 20-50% on every visit.

By becoming a UPC member, you’ll also gain lifetime access to:

  • Free 24/7 virtual care for off-hour questions and concerns

Team Member Emergency Fund

The Employee Association is a nonprofit organization that was formed in 2008 in an effort to aid Sun Health team members in times of need. The concept of the organization began when a team member approached management with a request to hold a bake sale to assist with fellow team members’ medical bills. With more than 400 team members on staff at that time, leaders saw a tremendous opportunity to create a formal plan to support our team members in need. As a result, the Employee Association was formed.

Over the years, the Association has brought relief and support to many team members in need and is sure to positively impact the lives of many more to come. The Employee Association is just one of the ways Sun Health shows its gratitude for our team members’ commitment to our mission to champion healthy living, research and superior health care.

Purpose

This association was created to provide monetary assistance to the members of the Sun Health Employee Association for circumstances such as the occurrence of any unexpected event or combination of circumstances that jeopardize the individual’s health and welfare.
Example of undue hardships include:

  • Offering assistance in time of need e. g. funeral expenses for the death of an immediate family member or the travel costs to see an ailing close relative.
  • Living expenses due to damage caused to team members dwelling
    (fire or storm)
  • Significant loss of household income due to events such as robbery or emergency medical expenses.

 How do I complete my enrollment?

Starting Monday, May 4th … To enroll or waive your benefits simply log into Ultimate Kronos Group (UKG).  You can log in from any computer that has internet access.
(We recommend not using a cell phone to complete the enrollment)
Once logged in to UKG, please navigate to the by clicking on the hamburger or 3 dashes in the upper left-hand side of screen.
Then go to “My Info > My Benefits > Enrollment > click on the “get started” in the Open Enrollment box.

IMPORTANT: You will need to go through each option and waive or elect as needed.  You will not be able to submit your elections unless all the benefits have a green check mark next to them. Your submittal will then come to the Benefits team to review and accept.  Enrollments or changes made will be effective July 1, 2026 – June 30, 2027.

Forgot password?  You can enter your Username & then click on the “forgot password” link and follow along from there.  You will need your cell phone number to help with the password reset.
As a reminder, your username for UKG is your FirstName.LastName + last 5 digits of your social security number (ex: jane.doe12345).
